Introduction: Bipolar II disorder comorbid with a history of heroin is associated with impairments in response inhibition and risky decision-making. The present study aimed to investigate the effectiveness of transcranial Direct Current Stimulation (tDCS) on response inhibition and risky decision-making in men with bipolar II disorder comorbid with a history of heroin. Methods: This was a quasi-experimental study with a pre-test, post-test, and follow-up design with a control group. The statistical population comprised men with bipolar II disorder comorbid with a history of heroin who were referred to treatment centers in Isfahan in 2024. A total of 32 individuals were randomly assigned to the experimental and control groups. The research instruments included the computerized Go/No-Go response inhibition task and the computerized Iowa Gambling Task (IGT). The experimental group received tDCS for 10 consecutive daily sessions, each lasting 20 minutes. Data analysis was performed using Analysis of Covariance (ANCOVA) via SPSS software. Results: The results indicated that transcranial Direct Current Stimulation significantly improved response inhibition—including commission error (F=29.31, P<0.001), omission error (F=28.56, P<0.001), and reaction time (F=24.47, P<0.001)—and reduced risky decision-making (F=27.71, P<0.001) in the experimental group compared to the control group. Conclusion: As a novel and effective intervention, transcranial Direct Current Stimulation can play a key role in reducing impulsive behaviors, facilitating the recovery process, and decreasing risky decision-making, making it a viable clinical application for individuals with bipolar II disorder comorbid with a history of heroin use.
