بررسی تاثیر داروی بومتاناید بر میزان توجه به اجزای چهره در کودکان و نوجوانان با اختلال طیف اوتیسم با استفاده از سیستم ردیاب چشمی

<div style="text-align: justify;"><strong><span style="font-family:B Titr;"><span style="font-size:12.0pt;">مقدمه:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> اختلال طیف اوتیسم</span></span><span style="color:#222222;"><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">، یک اختلال عصبی رشدی است که با نقص در ارتباطات اجتماعی و الگوهای رفتاری کلیشه­ای مشخص می­شود. </span></span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">نشان داده شده است که دارویی دیورتیک به نام بومتاناید، تاثیر<span style="color:#222222;"> قابل توجهی را </span></span><span style="font-size:14px;"><span style="color:#000000;">در بهبود ارتباط اجتماعی در این بیماران داشته است</span></span>. </span><span style="color:#222222;"><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">نظر به اینکه یکی از شاخص­های مهم در برقراری روابط اجتماعی، تماس چشمی می­باشد، هدف از انجام این مطالعه بررسی تاثیر این دارو بر تصحیح الکوی نگاه به چشم و دیگر اجزای صورت در کودکان و نوجوانان مبتلا به اختلال اوتیسم بود.</span></span></span><strong><span style="font-family:B Titr;"><span style="font-size:12.0pt;"></span></span></strong><br>
<strong><span style="font-family:B Titr;"><span style="font-size:12.0pt;">روش کار:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">. مطالعه حاضر یک کارآزمایی بالینی تک گروهی به صورت یک سو کور بود. شرکت­کنندگان شامل 20 کودک 7 تا 15 سال مبتلا به اختلال طیف اوتیسم با عملکرد بالا بود. بعد از قطع داروهای قبلی آزمون­های مقیاس درجه­بندی اوتیسم دوران کودکی (</span></span><span dir="LTR"><span style="font-family:Times New Roman,serif;">CARS</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">)، مصاحبه تشخیصی نیمه ساختاریافته (</span></span><span dir="LTR"><span style="font-family:Times New Roman,serif;">KSADS</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">)، معیار پاسخ­دهی اجتماعی (</span></span><span dir="LTR"><span style="font-family:Times New Roman,serif;">SRS</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">)، مقیاس تجدیدنظر شده هوشی برای کودکان (</span></span><span dir="LTR"><span style="font-family:Times New Roman,serif;">WISC-R</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">) برای هر شرکت­کننده انجام شد. مداخله دارویی روزی یک میلی­گرم قرص بومتاناید به صورت نیم میلی­گرم صبح و عصر به مدت نود روز انجام شد. تست ردیاب چشمی هنگام نگاه شرکت­کنندگان به تصاویر چهره قبل و بعد از مداخله انجام شد. متغیرهای تعداد و زمان نگاه</span></span> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">به هر منطقه در هر تصویر ثبت گردید. </span></span><br>
<strong><span style="font-family:B Titr;"><span style="font-size:12.0pt;">یافته­ها:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">مقایسه قبل و بعد از مداخله تعداد نگاه به چشم برای هر بیمار برای هر تصویر و زمان نگاه به چشم برای هر بیمار برای هر تصویر تفاوت معناداری را نشان نداد </span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">(05/0</span></span><span dir="LTR"><span style="font-family:Times New Roman,serif;">P></span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">). </span></span><br>
<strong><span style="font-family:B Titr;"><span style="font-size:12.0pt;">نتیجه­گیری:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">مصرف داروی بومتاناید بر روی میزان نگاه به چشم در کودکان اوتیستیک اثر معناداری نداشته گرچه </span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">علایم و شدت اوتیسم و برخی مشکلات رفتاری را بهبود داده است.</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"></span></span></div>
<strong>Introduction</strong>: Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder characterized by impairment in social communication and repetitive and stereotyped behaviors. It has been shown that a diuretic drug named bumetanide had a considerable influence on improving social communication in these patients. One of the important features in making social communication is eye contact; this research aimed to evaluate the efficacy of this medication on improving pattern of eye contact and the other parts of the face in children and adolescents with ASD. <br>
<strong>Methods: </strong>This single group clinical trial research is conducted on 20 children and adolescents aged from 7 to 15 years old with high function Autism Spectrum Disorder. After discontinuing the previously taking medications, Childhood Autism Rating Scale (CARS), Kiddie- Schedule for Affective and Schizophrenia (KSADS), Wechsler Intelligence Scale for Social Responsiveness Scale (SRS), and Children-Revised Form (WISC-R) were administered to each participant. A dose of 1 mg Bumetanide (0.5 mg in the morning and 0.5 mg in the evening) during 90 Days was given to the participants. Eye tracking evaluation administered while the participants were looking at the pictures of faces before and after the intervention. The number and duration of fixation to each area in each image as the variables recorded.<br>
<strong>Results:</strong> A comparison of pre and post-intervention showed no significant differences in terms of the eye tracking variables.<br>
<strong>Conclusion:</strong> Using Bumetanide did not have any significant effect on the amount of looking at the eyes in children and adolescents with ASD, despite improved symptoms and severity of autism and some behavioral problems.<br>
